Please give me as much information as possible on this form, and attach other pages as necessary.  This form expresses your vision and guidelines for the final piece, and each aspect will be carefully reviewed in our consultation, included in your customized ‘Contract to Commission’, and referenced throughout the painting process by the artist.  The details that you provide here are essential!  Send this form to the address at the top of the page, along with your photographs and any other inspirational or style ideas.  Please visit the Commission a Portrait page on our website and click  "uploading or sending photographs" for instructions on packaging and shipping your photographs.

1.  Tell me about the portrait that you’re envisioning.  How many subjects? What type of background?  Please attach the photograph(s) for consideration:

2.  Tell me about the subject(s) in the portrait.  Give me an idea of the personality and qualities that we’re trying to capture in each person or pet.  Please include any special details or stories that would help me to get a feeling for the person/pet or home.

3.  Are there special objects (such as clothing details/jewelry or special flowers/plants for home portraits) that you’d like to include?  Consider any symbolism or special details that might make the portrait even more special to your family.  Closer photographs of special clothing, jewelry, etc. are very helpful in establishing these details in the finished piece:

4.  Tell me about your style, favorite paintings/artist, etc.  Please feel free to send additional photos of your home and even of objects, ‘inspiration pieces’, or examples of the style of art that you like.  (I have based portrait style on everything from the Old Masters to the cover of a 1960’s baby book.):

5.  Where will you be hanging your portrait?  Please send a description and/or photographs, as well as the overall size restrictions if applicable:

Additional Requests:

Deckled edges on paper: A deckled edge is a soft uneven edge indicating a mould made paper.  With traditional matting, the edge is hidden behind the matting.  Deckled edges call for 'floating' the finished piece on top of the bottom mat, which enlarges the final size and gives your portrait a natural and artistic look.  Add Deckled edge: $20 for sizes up to 18", $50 for sizes up to 40"

Gallery Wrapped canvas (a modern treatment where the painting wraps around the deep sides of the canvas stretchers and does not require framing)

Additional Subjects: how many total subjects in your painting? ____
